Patient Information

We care about the time you spend with us, so to give you the proper amount of time for your visit, we prefer that you call our office to schedule an appointment.  Our office can be reached at 330-753-7772 during normal business hours.


However, if you have an urgent need to see a podiatrist during our office hours, we encourage you to come to the office.  We will do our best to work you into our schedule for that day. 


If you have a scheduled appointment and are unable to keep the appointment, we ask that you kindly notify us 24 hours in advance.


If you are having a medical emergency, call 911.

Helpful Reminders Before Your Appointment

We accept most insurance plans

Our office participates with most insurance plans.  If you are not sure if we accept your insurance, contact our office and we will be happy to assist you. 


If you do not provide us with your correct insurance information at the time of your visit, you will be responsible for the charges. 

Do you need a referral?

Some insurance plans require a referral from your family doctor.  Be sure to obtain this prior to your appointment with our office.

Bring a list of your medications

You may be prescribed a medication, if is helpful to have a list of your current medications so we can check for any interactions.  

Arrive 20 minutes early

If you are a new patient, or have not been to our office in over 3 years, you will need to fill out new paperwork.  Either print the paperwork from the site, or arrive 20 minutes early to fill it out in our office. 

Running Late?

We understand that things happen, but if you are running late, please be courteous can call the office.  If you arrive more than 15 minutes late, you may be asked to reschedule your appointment.

Copay

If you have a copay, it is due at the time of your visit.
